Things to think about to help you keep a diary for your PIP application

When you fill in the PIP application form, you have to say how much help you need to do some every day things.

A man writing in a diary at a desk

You might find it helpful to keep a diary of how much help you need to do these things every day for 2 weeks:

A lady slicing a cucumber
  • making a snack or a meal
A man sitting at a table is just about to eat a burger from a plate which has chips on it
  • eating and drinking
A man is holding up a packet of tablets trying to read the instructions
  • looking after your health condition and taking medication
A shower head with water coming out of it
  • washing and bathing or having a shower
A toilet
  • going to the toilet or managing your incontinence

Incontinence is when you can not make it to the toilet in time.

A woman is putting on a red cardigan
  • getting dressed and undressed
A woman talking to a man
  • talking and listening to other people and understanding what they are saying
A woman reading from a book.
  • reading
A group of teenagers
  • being with other people
A calculator next to a pile of money
  • looking after and spending your money
2 people walking away from a bus.
  • planning how to get somewhere and getting there
A girl in a moulded wheelchair smiling.
  • moving around.
An older man using a walking frame.

Write down how much help you needed.  Help might be special equipment to make it easier to do something.

A man smiling and looking at a younger man.

Help might be someone reminding you to do something.

A woman scratching her head and looking confused standing next to a question mark

Write down if it was easy or difficult to do the things.

A woman putting the palm of her hand on her head. She is wearing glasses and looks in pain or sad

Write down if doing any of the things made you feel tired, worried or upset.

A man in a wheelchair with his thumb down and holding a piece of paper with a red cross on it.

Write down if you had to stop doing any of the things, or if you could not do them and why.

You might want to send a copy of the information from your diary with your PIP application.

You do not have to share any personal information from your diary unless you want to.
