Step 1

You will be asked some questions. The answers you give will be built into the letter, like your address and date of birth.

Step 2

When you have answered all the questions you will be able to see and check the finished letter.

You can then:

• download your letter
• email the letter to yourself
• Get Mencap to print out the letter and post it to you.

Step 3

Take your letter with you to your next doctors appointment. You might be asked questions about your learning disability.

Step 4

When you are told that you have been added to the register, tell Mencap and join our I'm In campaign.
