If you choose to fill in this survey Mencap will share your answers with the Government’s team who are running the survey. You do not need to answer every question. 

You must live in England and be aged over 16 to take part.

You can ignore some questions if you do not want to answer them. You do not need to give your name or any personal details to take part.
